According to the University Student Code (Policy 553), Academic Appeals are accepted for the following: academic actions (including grades), exceptions to graduation, and general education requirements after they’ve reached the Dean level.

FERPA Consent to Release Form

The Family Educational Rights and Privacy Act (FERPA) of 1974 prohibits Utah Tech University from releasing certain personally identifiable information from a student’s record to a third party (e.g. parent, spouse, etc.) without the student’s explicit written consent. This form serves as written consent when properly completed.

Residency Status Update

For those seeking to register as residents of Utah for in state tuition, voting, etc. Applications for residency status are accepted no later than the end of the second week of the term reclassification is requested.

Exception to Policy petitions may be submitted for exceptions to the policies that guide registering or adding courses, dropping courses, auditing courses, payment of tuition and/or fees, refunds of tuition and/or fees, complete withdrawals, and other policies.
